Travel Release

Game Travel Release forms need to be filled out  when a student athlete will travel with their parent to or from a game.  Phillipsburg High School provides bus transportation for every contest.  If your child has an appointment, family or school obligation and must travel with the parent, please fill out the game travel release below and email it to [email protected] by the morning of the event.  (Athletes are NOT allowed to drive themselves to or from any contest). 

 

Practice/Home Game Travel Release forms need to be filled out  for all athletes that will be traveling to practice or a home game/match at Middle School fields/courts, Elementary School Pool/gym,  Warren Lanes or Architects  with a parent or driving themselves.  This is the only time an athlete is allowed to drive themselves.  This form should be emailed to the coach and [email protected] so it is kept on file in the athletic office.
